NYS Center for Excellence in Advance Care Planning

The process of Advance Care Planning (ACP) supports patient self-determination, decision making & quality end of life care. Planning for decisions about end-of-life care allows individuals to consider & express their values to govern their care, articulate factors important to them for quality of life & to clarify preferences with regards to specific interventions. Discussions also offer patients the opportunity to identify who will make decisions for them should they lack decision-making capacity. Proactive discussions with patients on their wishes if recovery from illness/injury is improbable & addressing concerns, expectations & misunderstandings about specific medical conditions/interventions is critical to secure patient choice. Encouraging patients to share their views ensures their values, goals & preferences on informed care decisions are honored.
