The Hospice and Palliative Care Association of New York State Infectious Disease Toolkit

HPCANYS developed this infectious disease toolkit to capture and build on lessons hospice and palliative care providers learned during the COVID-19 pandemic. It shares best practices for staff safety and training, personal protective equipment, and policies, to strengthen providers’ emergency management capability and capacity for effective management of familiar and emerging infectious diseases.

Identify, Isolate, Inform (3-Step Protocol)
“Identify, Isolate, and Inform” is a three-step approach used in health care to manage infectious disease cases.

Steps

• Identify: Recognize patients with signs and symptoms consistent with a communicable disease.
• Isolate: Separate patients to reduce exposure to others.
• Inform: Notify the appropriate internal and external contacts, following your organization’s protocols.
