| FUNCTION: | * Responsible for the proper staffing and coordination of the Referral Center to ensure adequate coverage to support admissions 7 days per week * Train support staff on referral and admission procedures; establish quality measures and documentation standards to meet Mission Quality and CMS protocols * Conduct audits and annual evaluations for direct reports * Assist VP of Patient Access with departmental goals and objectives. *Participate in the development, growth and support of the Palliative Care program. * As required, may function as an Admissions RN or On-call Supervisor * Must understand, follow and ensure compliance with all State and Federal regulations, and JCAHO standards and hospice conditions of participation |
| EDUCATION: | Valid NYS RN License CHPN certification, or will test within one year of eligibility |
| EXPERIENCE: | Minimum 3 years clinical nursing experience. Previous hospice/home care and case management experience highly preferred Excellent customer service skills Current NYS Driver’s License required |
