| FUNCTION: | Provide case management and nursing services to Hudson Valley Hospice patients and their families and facilitate relationships with nursing facilities, ensuring those services are consistent with the philosophy, goals, objectives and policies of the Agency. |
| EDUCATION: | NYS RN License |
| EXPERIENCE: | Minimum two years nursing experience Minimum one year medical/surgery experience Experience in Hospice, community health or oncology preferred Experience in caring for terminally ill patients and a basic understanding and commitment to the Hospice philosophy Reliable transportation and valid NYS Driver’s License |
