POSITION:                 Coordinator of Patient Services

DEPARTMENT/          Patient Services – Home Care Team

LOCATION:                Hudson Valley Hospice - Poughkeepsie, NY

STATUS/HOURS:       Full Time - 40 hours
                                    Monday - Friday / 8:30 a.m. - 4:30 p.m.
                                    Plus on-call supervisor rotation every 4 to 6 weeks; 24/7 x 7 days per week for both Dutchess and Ulster counties

FUNCTION:                Overall responsibility in conjunction with a hospice physician for the coordination of patient care and hospice eligibility for patients assigned to one of Hudson Valley Hospice’s IDG Teams. Ensures the provision of high-quality hospice care is delivered by Case Managers, Social Workers and others while maintaining compliance with professional standards, hospice standards, federal and state regulations, Joint Commission standards and the NYS Nurse Practice Acts. This position is responsible for providing support, supervision, professional leadership development and goal setting for Homecare Case Managers on the assigned IDG Team.

EDUCATION/             1. Graduate of an accredited school of nursing or social work. Current
EXPERIENCE:           registration as a registered nurse in New York State. Certified as a
                                    Hospice Palliative Care Nurse or obtain certification after one year
                                    of employment. BS or BSN (currently attending college to obtain a
                                    BSN) or commensurate experience, clinically and/or managerially.
                                    2.  Minimum of three (3) years of clinical nursing, preferably in oncology,
                                    and/or home care experience. At least one (1) year of which is medical-surgical nursing.
                                    3. Leadership and hospice experience preferred
                                    4. Valid NYS driver’s license
