South Nassau Communities Hospital provides palliative care services recognizing the physical, psycho-social, and spiritual needs of patients and their families with advanced life limiting conditions or illnesses and/or who are at end of life. The palliative care team collaborates with the interdisciplinary members of the South Nassau staff including physicians, nursing, social work, case management, spiritual care, pharmacy, nutrition and food services, physical therapy, occupation therapy and community agencies such as hospice to meet the needs of the patient and family as it pertains to relief of suffering and promotion of quality of life up to and including end of life. The team is comprised of a board certified hospice and palliative care physician, a advanced certified hospice and palliative care Nurse Practitioner, and an administrator who is also advanced certified hospice and palliative care social worker.
The palliative care team provides symptom assessment and management as well as enabling patients and families to make informed decisions about their care by education them on the process of the disease, prognosis, and the benefits and burdens of potential interventions and course of treatments.
